Heartland is an 80 bed long term care facility certified under both the Medicare and Medicaid programs. Heartland was purchased by Heartland Care, Inc., an affiliate of CHC in 2000. Heartland is the only nursing facility in a county of approximately 12,000 people. Heartland plays a vital roll in the community through various avenues including preparing the meals for Meals on Wheels. Heartland has also been recognized by the state licensing and certification as the model facility for their activity program.

Fair Oaks is very active in assisting with food drives for the local food banks as well as participating in Relay for Life.
